Students must apply and be accepted both to the college of their choice and CareerNext. Once all documents listed below have been submitted, your application will be reviewed by CareerNext.
Required Documents for CareerNext at Hudson Valley Community College and SUNY Cobleskill and CareerNext CHOICE at any U.S. college or university.
- CareerNext and CareerNext CHOICE Application
- Parent Form (not required for CareerNext CHOICE)
- High School IEP or 504 – if applicable
- Most recent psychological testing – if applicable
- High school transcript
- College transcript (if prior credits have been earned)
